የሴቶች አትሌቶች ምዝገባና የእድገት ቅጽ
ዓላማ: ይህ ቅጽ በፌዴሬሽኑ ውስጥ ለሴቶች አትሌቶች የተለየ ድጋፍ ለመስጠት እና የእድገት ጉዟቸውን በሳይንሳዊ መንገድ ለመከታተል ያገለግላል።
አጠቃቀም: አዲስ አትሌቶች የግል መረጃቸውን፣ የካራቴ ደረጃቸውን እና የወደፊት የአትሌቲክስ ግባቸውን በዚህ ቅጽ ይሞላሉ።
ጥቅም: አሰልጣኞች የአትሌቶችን የአካል ብቃት፣ የቴክኒክ ክህሎት እና የስነ-ልቦና እድገት ለመገምገም ይጠቀሙበታል።
የሥነ-ምግባርና የደህንነት ሪፖርት ቅጽ (Ethics and Safety Report Form)
ዓላማ: በዶጆ (Dojo) ውስጥ ወይም በውድድር መድረኮች የሚከሰቱ ማንኛውንም የስነ-ምግባር ጉድለቶች ወይም የደህንነት ስጋቶችን በግልጽ እና በአስተማማኝ ሁኔታ ለመዘገብ የሚያስችል ነው።
አጠቃቀም: ማንኛውም የፌዴሬሽኑ አባል ወይም አሰልጣኝ የትንኮሳ፣ የደህንነት ጥሰት ወይም ያልተገባ ድርጊት ሲያጋጥመው ይህንን ቅጽ በመሙላት ለሚመለከተው አካል ማሳወቅ ይችላል።
ሚስጥራዊነት: በቅጹ የሚቀርቡ መረጃዎች ሁሉ በሙሉ ሚስጥራዊነት ተይዘው ተገቢው እርምጃ ይወሰድባቸዋል።
